Introduction: Recently, the CRES+5 meeting was held as a follow-up to the Regional Conference on Higher Education for Latin America and the Caribbean, addressing the most significant challenges faced by Higher Education Institutions in contributing to sustainable development. These challenges particularly impact medical education, aligning with the demands of University 4.0 to guide the training of physicians amidst current demands and the severe health problems in the Latin American region. Objective: To analyze the most important challenges of medical education in Latin America, derived from CRES+5 and University 4.0, to contribute to the responsible fulfillment of its social mission. Methods: The study relied on document analysis and theoretical methods such as analysis-synthesis, induction-deduction, and systemic approaches to systematize the theoretical-conceptual references of the study subject. This was based on a review of specialized literature and the professional experience of the participating authors. Results and Discussion: Medical education must prioritize teacher preparation, a relevant curriculum, and a focus on science and innovation as essential pillars to ensure quality. These efforts should align with the principles of University 4.0 and Education 4.0, which promote strategies to train competent physicians capable of responsibly fulfilling their social mission.
